Yet another complication to the diabetic foot is the Charcot foot. This is a diabetic deformity. With Charcot foot the foot becomes painful, swollen and warm. If the foot is continued to be used for walking the result is a collapsed arch and the bottom of the foot becomes rocker shaped.

With sensory neuropathy the ability to regulate skin temperature and normal sweating is lost causing the skin to become dry and atrophic. When these abilities are lost cracks and fissures form putting the patient at risk for infections.

When the sensation is lost the diabetic patient is at risk for injury to the foot due to physical injury, chemical injury, and thermal injury. Without sensation often times these injuries are not treated. There are many things that a diabetic should be aware of that can cause foot injuries; walking bare foot, shoes that don't fit properly, thermal burns from heaters, hot pads, and water, and caustics burns due to over the counter medication to treat corns. Amputation is often the result of these injuries due to poor circulation.

Planter ulcers located on the bottom of the foot are caused from walking and or standing. Ulcers located on the top or sides of the foot are often due to pressure from the shoe.

Infection in the diabetic foot is difficult to treat due to poor circulation and decreased immune function. With infection the glucose control is impaired causing higher blood sugar levels and reducing immune defense mechanisms.

Foot Care

All people with diabetes need to wear shoes that protect the foot from injury. The diabetic patient should have shoes fitted by certified podiatrist. Shoes should be one half to five eights inch longer then the patient's longest toe. Heels should not be higher then 2 inches to keep the body weight from shifting and applying pressure to the metatarsal heads, increasing the risk of ulcers. Shoes that are too tight cause damage to the tissues if worn for more than 4 hours with out relief. Shoes should be changed mid day and evening. The patient with toe deformities will need to purchase custom shoes with wide and high toe boxes and extra depth. Those with Charcot foot will need molded shoes. All new shoes need to have a break in period where the patient periodically removes the shoes and visually inspects to feet for irritation and blisters.

Stocking and socks need to fit well and be appropriate for whatever activity is planned. Stockings should be free of seams, creases and holes to prevent irritation to the skin. Avoid stocking that are tight and have constricting bands.

There are also other ways to determine your foot type. Reason given: The sweet thing, by the name of Melanie Martinez, who is beloved by moms and kids alike, appeared in her ancient history as an actress in two videos spoofing public service announcements that advocate teenage sexual abstinence. Even PBS admits there was nothing pornographic about the videos.

(Why, since truth must be told, at least on this site, we ourselves just finished presenting an extended spoof of abstinence, which most responsible medical experts also feel obliged to ridicule as inadequate, in itself, to the problem.)

The lass is a 34-year-old who is married and the mother of a 3-year-old. As the host of "The Good Night Show," she introduced cartoons and demonstrated arts and crafts. "I had the best time making it," she said. "It was two glorious seasons that I filmed."

On a positive note, PBS received thousands of complaints about the dismissal, mostly from parents.

PBS's own ombudsman, Michael Getler, noted that firing her had "too much of a whiff of after-the-fact loyalty oaths and purity checks on performers who do lots of different things."

Do not the magnates at the head of PBS realize that the harsh justice meted out by them, who, as representatives of culture are obliged also to represent tolerance, sets an example for children that is far more deleterious than the Melanie Martinez way-back-when spoofs of a specious remedy for teen pregnancy.

Now, however, events have taken an even more distressing turn. Now PBS, conducting a review of all children's entertainment with the same moral ferocity, has alighted on Cinderella. They are incensed that she allowed herself to lose one of her glass slippers and be seen barefoot at a public occasion like a party.

Cinderella has claimed innocence on the ground that she lost the slipper when she was making a hasty exit so she could get home on time.

But the watchdogs at PBS have not been assuaged. Would such behavior meet with approbation in the most conservative regions and byways of contemporary America? Never! And no sense saying such moral rectitude is limited to a relatively small segment of the American public. Would her bare foot meet with approval in, for instance, a nation in the repressive grip of Muslim fundamentalism? Never!

As a result, they have issued a proclamation: "Cinderella and her ultimately racy story are hereby banished from PBS from this broadcast day forward. Out of consideration for the children who watch our especially infantile programs and their parents, who trust that our content will never assault their own moral sense, we can no longer permit a young woman who allowed herself to be seen in public VIVOBAREFOOT displaying a bare foot put even one toe on any stage on which PBS might produce a program for children. Therefore, Cinderella is hereby banned from any production funded by PBS. This proclamation is irreversible."

My, oh, my, what is a tolerant person to do when such appeasements of moral excess reach even as high as the cultural icons who conduct our public broadcasting system?

If you have ever been water skiing

If you have ever been water skiing, you most likely have struggled to learn or teach someone else. The real excitement begins when you try to learn to get up on combo skis, a slalom ski, a wakeboard, or even when you try to barefoot water ski. It doesn't take a genius to figure out that one way to dramatically accelerate the learning process is to use a bare foot boom.

The bare foot boom was originally designed to teach people to ski on your bare feet, but now it is used to minimize falling and injuries in all types of water skiing activities.

The boom is a round metal bar that extends from the boat perpendicular about 8-10 feet to either side. The boom creates a secure fixture that the skier can use for stability during the learning process. The boom dramatically increases skier confidence. If your boat's windshield ends in front of the pylon, you will need what is called a "straight boom." If your boom extends less than 8 inches past your pylon, then you need what is called a "contour boom." If your windshield extends more than 8 inches past your ski pylon, then you may need what is called a "quad boom." Since there are an incredible amount of variations in boat types, I recommend buying your boom from a dealer who guarantees that your boom will fit or they will buy it back

Make sure that you do not let the complexities of figuring out which bare foot boom you need stop you from enjoying the benefits of safety and fun that comes from using one in your water skiing activites. Water skiing is an addictive family activity when you experience the exhilaration of learning without falling.

I just happened to wake up early. Not knowing what to do, I decided to stroll on the grass in my lawn. I left my sleepers to a side.

There was no sun. Stars were winding up - up there in the sky. Wind was chilly as it was hitting the nape of my neck. I could sense the energy that the cold breeze was trying to infuse in me. I continued strolling... Slowly, I was surrounded by streams of fleeting thoughts so much so, that I forgot the need for any warmth around. I feel I am still a kid watching it change - under go metamorphosis, watching the time pass-by, with a finger between my teeth - bewildered, - in astonishment! How we used to live in our city then - in my childhood - how congested it used to be there! Now I have the vast open sky above and the ground below is part of the valley that I proudly call mine for the time being. How green this valley is! I now hear the noise coming from groups of children - playing football in the near by high school - learning biking on the curvy-curly roads with steep gradients - interrupted by the sound of a motor-vehicle every now and then - zipping by! How beautiful people are here!

Many images come to my mind now - images of their laughter, their smile, their folklore, their arts and crafts - how rich is their culture, how limited are their requirements, how contented are they! I feel the tickle below my feet. Birds are chirping their morning songs - cuck - cuck - chippy - chippy - churr - churr! Suddenly I am forced to return to the reality that many of the inhabitants of this place face day in and day out - that of the insurgency! How unrelated this beauty is to the family which has lost their only son in a terror-strike! Had there been not the problem of insurgency, this place would have been a great tourist destination. Sun is coming out now and before it dries the sap out of the dewy grass I have to return to my cottage and get into the daily routine once more.

Oh, what a feeling it was to walk VIVOBAREFOOT on the dew-y grass! How soothing it was to feel the pure unadulterated freedom of my soul! How nice it would be to become a child again! World moves in a rotation of routines it seems. We keep craving for the same old picture perfect world! A calming sensation of pure satisfaction! I bet you can't have a better start of a day!
